Commissioners for Oaths

The City Clerks department provides commissioning services Monday to Thursday from 8:30 a.m. to 4 p.m.

A commissioner is a person authorized by the Province of Ontario to take oaths or declarations of an affidavit or a statutory declaration.

A commissioner is not the same as a Notary Public. The City does not keep a list of Notary Publics; however Lawyers and Paralegals often provide this service.

The commissioner is not responsible for the content of the affidavit. The person signing the document is responsible for understanding the details of the document and ensuring the contents are accurate.

The person signing the document is responsible for ensuring they provide photo identification such as a valid Driver's Licence, Passport, Ontario Photo card and paying the $15 charge per signature for the service. The individual is responsible for supplying the form to be signed by the commissioner when they arrive at City Hall.

Documents we sign

  • sworn statement for the transfer of a used motor vehicle
  • domestic and foreign pension documents (Proof of Life)
  • legal Change of Name Form - Adult/Child
  • delayed statement of birth
  • statutory declaration for OSAP - Statement of common law status/supporting children
  • religious or conscientious belief vaccination exemption
  • invitation letters for a visitor's VISA to Canada

Documents we do not sign

  • domestic or foreign real estate
  • wills, living wills, codicils to wills, or estate settlement
  • court, legal or civil issues
  • divorce or separation, marriage or cohabitation documents (including support payment affidavit)
  • documents requiring a notary public’s signature
  • documents not in English or that do not have English descriptors

Important

You must sign affidavits in front of the commissioner. Please do not sign the document(s) prior.
